What is Bone Densitometry (DEXA/DXA)?

Bone densitometry, commonly referred to as a bone density scan, evaluates the structural strength of bone tissue and assesses the statistical risk of future fractures using quantitative, objective numerical data.

How Does Bone Densitometry Work?

This diagnostic modality utilizes DEXA (Dual-Energy X-ray Absorptiometry) technology:

  • Ultra-Low Dose Radiation: The system transmits X-ray beams at two distinct energy levels using an exceptionally low radiation dose. This exposure is significantly lower than that of a standard screening chest X-ray—roughly equivalent to less than one day’s worth of natural background radiation exposure.
  • Differential Absorption Measurement: Denser bone tissue absorbs a greater proportion of the X-ray photons. By calculating the exact amount of radiation that penetrates and exits the bone, the system quantifies the bone mineral density (BMD) in grams per square centimeter ($\text{g/cm}^2$).
  • Rapid Acquisition: The patient lies supine on a specialized scanning table, and the data acquisition is completed within 10 to 15 minutes. Scanning protocols focus primarily on the central skeleton, specifically the lumbar spine and the proximal femur (hip), as these anatomical sites carry the highest clinical risk for osteoporotic fractures.

Clinical Utility: What is it Used For?

Bone densitometry functions as both a definitive diagnostic tool and a quantitative risk assessment system:

  • Diagnosis of Osteoporosis: It serves as the gold standard and only definitive method to diagnose osteoporosis, a metabolic bone disease that causes a silent, progressive loss of bone mass.
  • Fracture Risk Stratification: It estimates the statistical probability of potential future fragility fractures.
  • Therapeutic Monitoring: It evaluates the longitudinal efficacy of anti-resorptive or anabolic bone medications in patients undergoing treatment for osteoporosis, tracking whether bone mineral density has stabilized or improved.

Interpretation of Results: Understanding the T-Score

The diagnostic classification is fundamentally based on the T-Score, which compares the patient’s BMD to that of a healthy, sex-matched young adult reference population at peak bone mass:

  • -1.0 and Above: Normal (Healthy bone density).
  • Between -1.0 and -2.5: Osteopenia (Low bone mass; bone density is diminished but has not yet crossed the threshold into osteoporosis. Clinical lifestyle or medical interventions should be considered).
  • -2.5 and Below: Osteoporosis (Severe loss of bone density; pharmacological treatment is clinically indicated).

Clinical Indications: Who Should Undergo a DEXA Scan?

  • All women aged 65 and older and all men aged 70 and older.
  • Postmenopausal women under age 65 who present with clinical risk factors for fracture (such as tobacco use, low body weight, or a maternal history of hip fracture).
  • Individuals who present with a loss of height greater than 4 cm or progressive kyphosis (postural changes).
  • Patients undergoing long-term systemic corticosteroid (cortisone) therapy.
  • Individuals who sustain a low-trauma fragility fracture (a bone break resulting from a minor impact or fall from standing height).
