What Is Thyroid Cancer?

Thyroid cancer begins when cells in the thyroid grow and divide abnormally. The thyroid is a small, butterfly-shaped gland at the front of the neck that helps control the body’s energy use. There are several types of thyroid cancer, and their behavior can vary from slow-growing to aggressive. Many forms are treatable, especially when found early.

Symptoms

Thyroid cancer is often found as a painless thyroid nodule or lump in the neck. Early disease may cause no noticeable symptoms, and it may not change thyroid hormone levels. Many thyroid nodules are benign, but a persistent or changing finding needs medical assessment.

  • A new or growing lump may appear in the front or side of the neck.
  • Persistent neck swelling or fullness may develop around the thyroid.
  • Hoarseness or another lasting voice change can occur.
  • Difficulty swallowing may cause a sensation that food is sticking.
  • Difficulty breathing or pressure in the neck may occur.
  • Persistent pain may be felt in the neck, throat, or ears.

These signs have many possible causes and do not by themselves prove cancer. Arrange a clinical assessment for a lump that persists, grows, or changes, especially with voice, swallowing, or breathing problems.

What Causes Thyroid Cancer?

Thyroid cancer develops when genetic changes cause thyroid cells to grow and divide abnormally. In many cases, the exact cause is unknown. Some changes are inherited, while others develop during a person’s lifetime. A thyroid nodule or nodules on the thyroid do not automatically mean that cancer is present.

  • Radiation exposure to the head or neck can increase risk, especially during childhood.
  • Inherited gene changes can cause some medullary thyroid cancers and related syndromes.
  • A family history may indicate increased susceptibility to certain thyroid cancers.
  • Most thyroid nodules are not cancerous and do not mean that cancer is present.

CANNOT BE CHANGED

Risk is associated with age, sex, family history, and inherited gene changes. Inherited syndromes are particularly important in some cases of medullary thyroid cancer or medullary thyroid carcinoma.

EXPOSURE AND MEDICAL FACTORS

Previous radiation to the head or neck can raise risk, especially in childhood. Certain thyroid conditions, iodine-related factors, and other medical exposures may matter in selected cases, but they are not direct causes for everyone.

Having a risk factor does not mean that someone will develop thyroid cancer. Many people who are diagnosed have no obvious risk factor, and most thyroid nodules are benign.

Report concerning changes promptly

Tell your care team about new breathing or swallowing difficulty, worsening neck swelling, persistent voice changes, or tingling, muscle cramps, or other possible symptoms of low calcium. Treatment may cause low thyroid hormone after thyroid removal, calcium or parathyroid problems, voice changes after surgery, fatigue, or effects from radioactive iodine.

  • A clinician examines the thyroid and nearby lymph nodes and reviews symptoms and risk factors.
  • Blood tests assess thyroid hormone function but do not by themselves diagnose cancer.
  • Ultrasound evaluates the nodule’s size and features and checks nearby lymph nodes.
  • Fine-needle aspiration may collect cells for examination by a pathologist.
  • Additional scans or molecular tests may be used when the diagnosis or extent of disease is unclear.

SURGERY

Part or all of the thyroid may be removed, with lymph-node surgery when indicated. Selected low-risk cancers may instead be monitored with active surveillance.

RADIOACTIVE IODINE

Radioactive iodine is used for selected differentiated thyroid cancers after surgery or for recurrent disease.

MEDICINES AND RADIATION

Hormone replacement, targeted medicines, external radiation, chemotherapy, or other systemic therapy may be used according to the cancer type and extent.

Papillary thyroid cancer, follicular thyroid cancer, medullary thyroid cancer, and anaplastic cancer can require different treatment plans. A multidisciplinary cancer team may consider the tumor’s size, stage, molecular features, symptoms, overall health, and personal preferences.

Outlook and Prognosis

Outlook and Prognosis

Prognosis varies substantially by the type of thyroid cancer and how far it has spread. Many papillary and follicular thyroid cancers are highly treatable, while medullary, anaplastic, recurrent, or metastatic disease may require more intensive care. Doctors also consider tumor features, age, response to treatment, and recurrence risk.

Thyroid cancer can recur even after successful treatment, so ongoing monitoring is important. Use your survivorship and follow-up plan to understand which tests are needed and what your individual outlook means.

  • Attend follow-up visits on the schedule set by the oncology or endocrinology team.
  • Take thyroid hormone replacement exactly as prescribed when it is needed.
  • Use blood tests and neck imaging as recommended to monitor treatment response or recurrence.
  • Report a new neck lump, persistent voice change, swallowing difficulty, or unexplained symptoms.
  • Ask about bone health, heart health, calcium balance, and emotional support when long-term treatment affects these areas.
