Vaginal pH Testing

One of the fastest and most accurate ways to categorize an infection is by testing the acidity of the vagina. A healthy vagina typically has a pH between 3.8 and 4.5.

  • Yeast Infections: Usually maintain a normal, acidic pH.
  • BV and Trichomoniasis: Typically cause the pH to rise above 4.5. Using a simple pH strip during the exam provides immediate clues about which microbes have taken over the environment.

The "Wet Mount" (Microscopic Analysis)

The “wet mount” is the gold standard of vaginal diagnosis. A sample of discharge is placed on a slide with a saline solution and viewed under a microscope. The doctor looks for three key markers:

  • Clue Cells: Vaginal skin cells coated in bacteria, a definitive sign of Bacterial Vaginosis.
  • Yeast Buds/Hyphae: Visible fungal structures that confirm Candidiasis.
  • Trichomonads: Small, moving parasites that confirm Trichomoniasis.

The Whiff Test (KOH Preparation)

  • A specialized test involves adding potassium hydroxide (KOH) to a sample of vaginal discharge. If this produces a strong, fish-like odor, it is a positive “whiff test,” which is a primary indicator of Bacterial Vaginosis.
    The KOH also helps dissolve other cellular material on the slide, making yeast structures even easier to see under the microscope.

Nucleic Acid Amplification Tests (NAAT)

For persistent or complex cases, modern laboratories use NAAT technology. This highly sensitive DNA-based test can detect the genetic material of specific bacteria and parasites.
NAAT is particularly useful for identifying “co-infections,” where a patient may have both a common infection and an underlying STI like Chlamydia or Gonorrhea that could be missed by a simple microscope check.

Vaginal Cultures

If an infection is recurrent or does not respond to initial treatment, a vaginal culture may be performed. The sample is sent to a lab where the organisms are grown in a controlled environment.
This allows specialists to determine the exact strain of yeast or bacteria and most importantly which specific medications that strain is sensitive to.
This “sensitivity testing” is the key to breaking the cycle of chronic infections.

Screening for Underlying Conditions

If a woman suffers from frequent yeast infections, the evaluation may expand to include a blood glucose test.
Undiagnosed or poorly managed diabetes can lead to high sugar levels in vaginal secretions, which acts as a constant food source for yeast.
Identifying these systemic links ensures that the treatment covers the whole body, not just the localized symptoms.

Cervical Screening (Pap Smear)

While a Pap smear is primarily used to screen for cervical cancer and HPV, it can sometimes reveal the presence of certain inflammatory cells or infections. If you are due for your routine screening, your doctor may combine the diagnostic workup for your infection with your Pap smear to provide a comprehensive assessment of your reproductive health.

Evaluating Hormonal Cytology

For women in the perimenopausal or postmenopausal stages, the specialist may evaluate the “maturation index” of the vaginal cells.
