Before surgery becomes strictly necessary, there is a period of “watchful waiting.” During the early stages of a cataract, medical management focuses on improving your quality of life without an operation.

Prescription Updates: A new pair of glasses can often improve vision temporarily by correcting the nearsightedness caused by the cataract’s swelling.
Lighting Aids: Using brighter LED lamps for reading can help overcome the dimming effect of the lens.
Glare Control: Wearing polarized sunglasses and using anti-glare coatings on glasses helps manage sensitivity to sunlight and headlights.
The Reality: These are temporary fixes. Once the cataract surgery treatment options become necessary, meaning the vision loss interferes with driving, reading, or safety, surgery is the only path forward.

Minimally Invasive Phacoemulsification

The global standard for removing cataracts is Phacoemulsification, often called “Phaco.” It is a marvel of minimally invasive medicine and is the foundation of our surgical care.

The Incision: The surgeon makes a microscopic incision (about 2.2 mm) at the edge of the cornea. It is so small that it is self-sealing and rarely requires stitches.
The Ultrasound: A tiny probe is inserted into the eye. It emits ultrasound waves that vibrate at high speed to break the cloudy lens into tiny fragments.
The Removal: The probe gently vacuums out the pieces.
The Implant: The artificial lens (IOL) is folded up like a taco, inserted through the tiny cut, and unfurled inside the eye to sit permanently in place.

Femtosecond Laser-Assisted Surgery (FLACS)

For patients seeking the highest level of precision, LIV Hospital offers Femtosecond Laser-Assisted Cataract Surgery, which is completely blade-free. Instead of a hand-held blade, a computer-guided laser creates the incisions, ensuring each cut is perfectly precise and consistent. The laser opens the capsule holding the lens in a perfect circle, allowing the new lens to sit exactly in the optical center of the eye, which is essential for premium optics. It also pre-softens the cataract, reducing the ultrasound energy needed for removal, minimizing stress on delicate eye tissues, and speeding up recovery.

Intraocular Lens (IOL) Options

Choosing the right lens is critical. Monofocal lenses are the standard option and are set to focus at one distance, usually far, giving excellent distance vision but requiring reading glasses for close tasks. Toric lenses are designed for patients with astigmatism and correct the eye’s “football shape” to provide clear distance vision, with digital alignment systems ensuring perfect placement. Trifocal “Smart” lenses are premium lenses that split light to provide clear vision at near, intermediate, and far distances, allowing most patients to reduce or eliminate their dependence on glasses.

What to Expect After Treatment

Cataract surgery is an outpatient procedure, meaning you do not stay in the hospital overnight.

Immediate Post-Op: Your vision will be foggy, similar to looking through a steamy bathroom mirror or water. This is normal.
Protection: You may wear a clear plastic shield over your eye while sleeping for the first week. This prevents you from accidentally rubbing your eye while you sleep.
Sensation: It is common to feel a scratchy sensation, like a grain of sand is in the eye. This usually resolves within 24 hours.
Visual Recovery: Most patients notice a dramatic improvement in brightness and color within the first 24 to 48 hours. The “fog” clears rapidly.

Rehabilitation and Neuro-Adaptation

While the eye heals physically in a few weeks, the brain may take longer to adjust to the new vision. This process is called neuro-adaptation.

Color Changes: The world will look surprisingly bright and blue. Your old cataract acted like a yellow filter, blocking blue light. Removing it lets the full spectrum in again.
Positive Dysphotopsia: You might see a shimmering arc of light or a shadow in your peripheral vision. This is a reflection off the edge of the new lens. It is harmless and usually disappears as the brain learns to ignore it.
Adapting to Trifocals: If you choose Smart Lenses, your brain needs to learn how to switch focus between the near and far rings of the lens. This can take 1 to 3 months of “brain training.”

Long-Term Management and Follow-Up
