To understand how this Smart Drug works, imagine a tumor is like a tiny, growing city. To survive and expand, this city needs a constant supply of food and oxygen. It gets these by building its own network of new “pipelines”—which are actually blood vessels. This process is called angiogenesis.

At the molecular level, fruquintinib works as a “pipeline blocker”:

  1. Spotting the Antennas: Cancer cells send out a protein called VEGF. This protein looks for special antennas on the surface of blood vessel cells called VEGFR-1, -2, and -3.
  2. Blocking the Signal: Fruquintinib travels through the bloodstream and sits directly on these VEGFR antennas. It acts like a cap, preventing the VEGF protein from “plugging in.”
  3. Stopping the Growth: Because the signal is blocked, the internal “wiring” of the cell (signaling pathways like the MAP kinase pathway) is turned off.
  4. Starving the Tumor: Without these signals, the tumor cannot build new blood vessels. The existing vessels may even shrink. Without a fresh blood supply, the tumor starves, stops growing, and its spread is slowed down.

Fruquintinib is specifically approved for patients with advanced stages of cancer that have spread to other parts of the body.

Oncological Uses

  • Metastatic Colorectal Cancer (mCRC): For adult patients who have been previously treated with fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and, if medically appropriate, an anti-EGFR therapy.

Non-Oncological Uses

  • None. This medication is strictly used for the treatment of cancer.

Dosage and Administration Protocols

Fruquintinib is taken as a capsule at home. It is important to follow a specific “on and off” schedule to give the body time to rest.

  • Overall Survival: In the global FRESCO-2 trial, patients taking fruquintinib lived significantly longer than those taking a placebo. The numerical data showed a median overall survival of 7.4 months for fruquintinib compared to 4.8 months for the control group.
  • Progression-Free Survival: The drug effectively doubled the time patients lived without their cancer getting worse. The median time was 3.7 months for fruquintinib versus 1.8 months for the placebo.
  • Disease Control: Approximately 56% of patients achieved “disease control,” meaning their tumors either shrank or stayed the same size for a period of time.

Safety Profile and Side Effects

Because fruquintinib affects blood vessels, it can cause specific side effects related to blood pressure and skin.

Black Box Warning:

Fruquintinib does not currently have an FDA Black Box Warning. However, it carries serious warnings for severe hypertension (high blood pressure) and severe bleeding.

Common Side Effects (>10%)

  • Hypertension: High blood pressure is the most common side effect.
  • Hand-Foot Skin Reaction: Redness, pain, or swelling on the palms of the hands and soles of the feet.
  • Fatigue: Feeling unusually tired or weak.
  • Diarrhea: Frequent or loose stools.
  • Abdominal Pain: Stomach aches or discomfort.
  • Hypothyroidism: Low thyroid hormone levels.

Serious Adverse Events

  • Gastrointestinal Perforation: A rare but serious hole in the wall of the stomach or intestines.
  • Severe Bleeding (Hemorrhage): Dangerous bleeding in the brain or digestive tract.
  • Posterior Reversible Encephalopathy Syndrome (PRES): A rare brain condition causing headaches, seizures, and confusion.

Management Strategies

  • For High Blood Pressure: Your doctor will check your blood pressure weekly. You may need to start or adjust blood pressure medicine.
  • For Hand-Foot Reaction: Use thick, fragrance-free moisturizers and avoid hot water on your hands and feet.

Research Areas

Fruquintinib is a major focus in Combination Immunotherapy research. Scientists are currently testing if blocking blood vessel growth with fruquintinib can make “checkpoint inhibitors” (a type of immunotherapy) work better. The theory is that by normalizing the blood vessels around a tumor, the body’s natural killer T-cells can enter the tumor more easily to destroy it. Studies are also ongoing to see if this drug can help in gastric (stomach) and lung cancers.

  • Do monitor your blood pressure at home every day and keep a log.
