Drug Overview

Huaier extract granule (also known as Trametes robiniophila Murr. extract) is a traditional Chinese medicine (TCM) derived from the medicinal fungus Huaier, which grows on the trunks of old pagoda trees. In modern oncology, it is processed into an aqueous extract granule and used as an adjuvant (supplemental) therapy to treat a variety of solid tumors, most notably hepatocellular carcinoma (HCC) and breast cancer.

Approved by the Chinese State Food and Drug Administration (SFDA) as early as 1992, Huaier extract is increasingly recognized in Western clinical research for its ability to reduce cancer recurrence and improve survival rates when combined with standard-of-care treatments like surgery, chemotherapy, or ablation.

  • Generic Name: Huaier extract granule.
  • Drug Class: Botanical Antineoplastic / Biological Response Modifier.
  • Primary Components: Proteoglycans (41.5% polysaccharides, 12.93% amino acids).
  • Route of Administration: Oral (Granules dissolved in warm water).
  • Current Status: Approved in China for liver, breast, and lung cancers; Investigational/Supplemental globally. As of March 2026, it is the subject of several international Phase IV and prospective trials.

What Is It and How Does It Work? (Mechanism of Action)

Huaier extract granule is a “multi-target” therapy that affects both the tumor cells directly and the body’s immune environment.

1. Direct Anti-Tumor Activity

Huaier interferes with the basic machinery of cancer cell growth.

  • Cell Cycle Arrest: It induces arrest at the G0/G1 checkpoint, preventing cancer cells from replicating their DNA and dividing.
  • Apoptosis Induction: It triggers programmed cell death by activating the Caspase pathway and modulating the Bcl-2/Bax ratio.
  • Anti-Angiogenesis: It inhibits the expression of VEGF (Vascular Endothelial Growth Factor), effectively “starving” the tumor by preventing the formation of new blood vessels.

2. Immune System Modulation

Unlike chemotherapy, which can suppress the immune system, Huaier acts as an immune booster.

  • T-cell Activation: It increases the percentages of CD3+ and CD4+ T-cells, as well as the CD4+/CD8+ ratio.
  • Natural Killer (NK) Cell Enhancement: It boosts the activity of NK cells, which are the body’s first line of defense in identifying and killing “stealthy” cancer cells.

3. Signaling Pathway Inhibition

Molecular research highlights its ability to block key survival signals like the PI3K/AKT/mTOR and Wnt/$\beta$-catenin pathways, which are often overactive in aggressive cancers.

FDA and Global Clinical Indications

Huaier is approved for clinical use in China but remains a “complementary” or “investigational” agent in the U.S. and Europe.

Current clinical applications supported by 2024–2026 research include:

  • Hepatocellular Carcinoma (HCC): Used after surgical resection or microwave ablation to significantly reduce the risk of recurrence and prolong overall survival.
  • Early and Advanced Breast Cancer: Used as adjuvant therapy to improve Disease-Free Survival (DFS), particularly in Triple-Negative Breast Cancer (TNBC).
  • Gastrointestinal Cancers: Studied in combination with chemotherapy for gastric, colorectal, and pancreatic cancers.
  • Lung Cancer: Investigated for its ability to reduce tumor markers and improve quality of life in patients undergoing radiation or chemotherapy.

Dosage and Administration Protocols

Because Huaier is an extract granule, it is typically dissolved in warm water and taken orally. Dosage varies based on the severity of the disease and the specific treatment protocol.

Treatment DetailResearch & Clinical Specification
Standard Adjuvant Dose20g taken orally, 3 times per day.
Maintenance/Prevention10g to 20g taken orally, 1 to 3 times per day.
AdministrationDissolve granules in approximately 100-200mL of warm water.
TimingOften started concurrently with or immediately following chemotherapy/surgery.
DurationTypically administered for 6 months to 2 years to prevent recurrence.

Clinical Efficacy and Research Results

Landmark studies published between 2018 and 2026 have solidified Huaier’s place in evidence-based oncology.

  • The Liver Cancer Breakthrough: A massive multicenter trial of 1,044 patients (published in Gut) showed that Huaier increased the Recurrence-Free Survival (RFS) rate from 49% to 62% in liver cancer patients after surgery.
  • Triple-Negative Breast Cancer (TNBC): 2024/2025 research indicates that Stage III TNBC patients receiving Huaier granules achieved a 5-year DFS of 81.3% compared to only 53.8% in the control group.
  • Reduction of Tumor Markers: Prospective trials in 2025 (NCT05871437) confirmed that Huaier significantly accelerates the return of tumor markers (CEA, CA125, CA153) to normal levels following breast cancer surgery.
  • Quality of Life (QoL): Consistently across trials, patients taking Huaier report higher Karnofsky Performance Scale (KPS) scores and lower incidences of anxiety and emotional distress compared to those on standard therapy alone.

Safety Profile and Side Effects

Huaier extract is remarkably well-tolerated, with a safety profile far superior to that of cytotoxic chemotherapy.

Common Side Effects:

  • Gastrointestinal Issues (Rare): Mild diarrhea or loose stools. This often resolves as the body adjusts or if the dose is temporarily reduced.
  • Palatability: Some patients find the earthy, mushroom-like taste of the dissolved granules difficult to tolerate over long periods.

Serious Risks:

  • None Significant: Clinical trials involving thousands of patients have reported no Grade 3 or 4 (severe) toxicities directly attributable to Huaier granules.
  • Drug Interactions: There is no evidence that Huaier interferes with the efficacy of common chemotherapies like capecitabine or tamoxifen.

Research Areas

In the fields of Stem Cell and Regenerative Medicine, Huaier is a major subject for studying “Cancer Stem Cell (CSC) Reversal.” 2026 research focuses on how Huaier polysaccharides can force dormant breast cancer stem cells to “differentiate” (mature), making them vulnerable to chemotherapy. By clearing the stem cell “niche,” Huaier aims to eliminate the “seeds” of cancer that cause late-stage relapses years after the initial tumor is removed.

Patient Management and Practical Recommendations

Pre-treatment Tests:

  • Baseline Tumor Markers: (CEA, CA19-9, AFP) to monitor the drug’s “normalizing” effect.
  • Liver & Kidney Function: While Huaier is safe, baseline checks are standard to ensure proper metabolism of any botanical extract.

“Do’s and Don’ts”:

  • DO be consistent. The benefits of Huaier in preventing recurrence are cumulative and require at least 6 months of continuous use.
  • DO dissolve the granules thoroughly in warm (not boiling) water to preserve the bioactive proteoglycans.
  • DON’T stop standard therapy. Huaier is an adjuvant and should be used to enhance, not replace, surgery or chemotherapy.
  • DON’T worry about “immune over-activation.” Unlike immune checkpoint inhibitors, Huaier functions as a balancer (immunomodulator) and does not typically cause autoimmune attacks on healthy organs.
