Drug Overview

Shark cartilage extract AE 941 is an experimental liquid medication derived from the cartilage of sharks. It was developed based on the scientific observation that shark skeletons, which are made entirely of cartilage, rarely develop tumors. Researchers created this standardized extract to see if the same protective properties could be used to help humans fight cancer and other diseases involving abnormal blood vessel growth.

This substance is classified as a “Smart Drug” because it is designed to interfere with specific biological processes that tumors need to grow, rather than attacking all cells in the body. While it has been studied extensively in clinical trials, it is not currently part of standard medical practice in the United States or Europe.

  • Generic name: Shark cartilage extract AE 941 (also known as Neovastat)
  • US Brand names: None (Investigational agent)
  • Drug Class: Angiogenesis inhibitor, Matrix Metalloproteinase (MMP) inhibitor
  • Route of Administration: Oral (taken by mouth as a liquid solution)
  • FDA Approval Status: Investigational (Not currently approved by the FDA for standard medical use)

What Is It and How Does It Work? (Mechanism of Action)

Shark cartilage extract AE 941 acts as a Targeted Therapy. To understand how it works at the molecular level, we must look at how a tumor tries to survive. A tumor is like a small city that needs food and oxygen to grow. To get these supplies, the tumor sends out signals to grow its own private “plumbing system” of new blood vessels. This process is called angiogenesis.

AE 941 works through three main molecular pathways to stop this “plumbing” from being built:

  1. Blocking VEGF: The extract interferes with a protein called Vascular Endothelial Growth Factor (VEGF). VEGF is the primary signal a tumor uses to tell nearby blood vessels to grow toward it. By blocking the VEGF receptor signaling, AE 941 prevents the blood vessels from receiving the message to grow.
  2. Inhibiting MMPs: Tumors use special tools called Matrix Metalloproteinases (MMPs) to chew through the healthy tissue around them so they can spread. AE 941 blocks these MMP enzymes, specifically MMP-2, MMP-9, and MMP-12. This makes it harder for the tumor to expand its territory.
  3. Activating Angiostatin: The extract may stimulate the production of angiostatin, a natural protein in the body that actively prevents new blood vessels from forming.

By cutting off the tumor’s supply lines and blocking its tools for expansion, AE 941 aims to “starve” the cancer and prevent it from spreading to other organs.

FDA Approved Clinical Indications

Because shark cartilage extract AE 941 is an investigational drug, it does not have official FDA-approved uses for the general public. It has been studied in clinical trials for the following conditions:

Oncological uses

  • Investigational treatment for advanced Non-Small Cell Lung Cancer (NSCLC).
  • Investigational treatment for metastatic Renal Cell Carcinoma (kidney cancer).
  • Investigational treatment for various solid tumors where blood vessel growth is a factor.

Non-oncological

  • Investigational treatment for Psoriasis (a skin condition involving excessive blood vessel growth).
  • Investigational treatment for Age-Related Macular Degeneration (an eye condition).

Dosage and Administration Protocols

In clinical research settings, AE 941 was administered as a flavored liquid that the patient would drink. The following table represents the standard dosing used in Phase 2 and Phase 3 clinical trials.

Treatment PhaseStandard Investigational DoseFrequency of AdministrationRoute of Administration
Phase 3 Clinical Trials30 milliliters to 60 millilitersTwice dailyOral (Liquid)

Dose Adjustments:

There are no established dose adjustment guidelines for renal (kidney) or hepatic (liver) insufficiency for this extract. However, because the kidneys filter components of the extract, doctors usually monitor kidney function blood tests regularly. If a patient experiences severe digestive distress, the dose may be temporarily lowered.

Clinical Efficacy and Research Results

Clinical research data from 2020 to 2025 has primarily focused on reviewing the long-term results of major Phase 3 trials, such as the LURIC and P-02 trials.

Numerical data from these studies showed that when AE 941 was added to standard radiation and chemotherapy for lung cancer, it did not significantly increase the overall survival rate compared to patients who only received standard care. In lung cancer trials, the median survival for patients receiving the extract was approximately 14 months, which was nearly identical to the group not receiving it. However, in trials for kidney cancer, researchers noted that a small group of patients receiving very high doses showed a slight delay in the time it took for the disease to progress. Current research has mostly moved toward combining similar angiogenesis inhibitors with modern immunotherapy, as the extract alone was not powerful enough to replace standard treatments.

Safety Profile and Side Effects

Shark cartilage extract AE 941 is generally considered to have a low toxicity profile compared to traditional chemotherapy.

Black Box Warning:

There is no official FDA Black Box Warning for shark cartilage extract AE 941.

Common side effects

These side effects are reported in more than 10 percent of patients:

  • Nausea and vomiting
  • Fishy aftertaste or bad taste in the mouth
  • Stomach bloating or gas
  • Diarrhea
  • Feeling very tired (fatigue)

Serious adverse events:

  • Significant changes in liver function tests (indicating liver stress)
  • Severe allergic reactions (hives, swelling, or trouble breathing)
  • Hypercalcemia (dangerously high levels of calcium in the blood)

Management strategies:

The most common problem for patients is the taste and smell of the liquid. To manage this, doctors often suggest chilling the liquid or mixing it with fruit juice. For nausea, taking the dose with a light meal may help. If blood tests show that calcium levels are rising too high, the treatment must be stopped immediately to protect the heart and kidneys.

Research Areas

Shark cartilage extract AE 941 is often discussed in Research Areas related to natural compounds and “metabolic” cancer therapy. While it is not a part of stem cell therapy, scientists are interested in how the anti-angiogenic properties of the extract could be used in regenerative medicine to prevent unwanted blood vessel growth after certain types of surgeries or in chronic inflammatory diseases. Currently, the most active area of research is determining if the specific enzymes found in shark cartilage can be synthesized in a lab to create even more precise “Smart Drugs” for the future.

Patient Management and Practical Recommendations

Pre-treatment tests to be performed:

  • Blood tests to check baseline calcium levels.
  • Liver function tests (ALT, AST, and Bilirubin).
  • Kidney function tests (Creatinine and BUN).
  • Imaging (such as a CT scan) to measure the size of the tumor before starting.

Precautions during treatment:

Patients with a known allergy to fish or shellfish should use extreme caution, as the extract is derived from marine life. Because this is an investigational product, it should never be used as a replacement for treatments proven to work, such as surgery or chemotherapy.

Do’s and Don’ts list:

  • Do keep the liquid refrigerated to help with the strong taste.
  • Do report any new skin rashes or yellowing of the eyes to your doctor immediately.
  • Do stay hydrated to help your kidneys process the extract.
  • Don’t take extra calcium or vitamin D supplements without asking your doctor first.
  • Don’t use this extract if you are pregnant or breastfeeding, as it may interfere with the development of blood vessels in a baby.
  • Don’t buy “shark cartilage” over the counter and assume it is the same as AE 941; the clinical extract is highly purified and standardized.
