What Are the Treatment Options for Infertility, and How Are They Chosen?

Modern reproductive medicine offers a wide range of treatments, from medication to advanced laboratory techniques, tailored to the cause of infertility, age, and time trying to conceive.

Ovulation Induction

GYNECOLOGY

For women who have irregular cycles or do not ovulate at all, often due to conditions like Polycystic Ovary Syndrome (PCOS), medication is the first line of defense. The goal is to stimulate the ovaries to release one or more eggs.

  • Oral Medications: Drugs like clomiphene citrate and letrozole are commonly prescribed. They work by tricking the brain into producing more follicle-stimulating hormone (FSH), which encourages egg growth.
  • Injectable Hormones: If oral meds fail, stronger hormones called gonadotropins may be used. These are injected daily and require close monitoring with ultrasound to prevent multiple pregnancies.

Intrauterine Insemination (IUI)

GYNECOLOGY

IUI, often called artificial insemination, is a procedure where sperm is placed directly inside the uterus around the time the ovary releases one or more eggs. This shortens the journey for the sperm and bypasses any potential barriers in the cervix.

The process involves preparing the sperm in the lab to concentrate the healthiest, most motile sperm. It is often combined with ovulation induction medication to ensure timing is perfect. IUI is frequently used for couples with unexplained infertility, mild male factor infertility, or issues with cervical mucus.

In Vitro Fertilization (IVF)

IVF is the most effective form of assisted reproductive technology. It involves retrieving mature eggs from the ovaries and fertilizing them with sperm in a laboratory dish.

  • Ovarian Stimulation: You take injectable hormones for 8 to 14 days to produce multiple eggs.
  • Egg Retrieval: A minor surgical procedure is performed under sedation to collect the eggs.
  • Fertilization: The eggs are mixed with sperm. In cases of male infertility, a single sperm is injected directly into the egg (ICSI).
  • Embryo Transfer: One or two healthy embryos are transferred into the uterus. Any remaining high-quality embryos can be frozen for future use.

Surgical Interventions

Sometimes, physical barriers prevent conception. Surgery can repair these issues and restore natural fertility or improve the success rates of IVF.

  • Laparoscopy: This minimally invasive surgery treats endometriosis, removes ovarian cysts, and opens blocked fallopian tubes.
  • Hysteroscopy: This procedure removes polyps, fibroids, or scar tissue from inside the uterus that might be preventing an embryo from implanting.

Care also extends to protecting your future fertility. Egg freezing is an option for women who wish to delay childbearing for career or personal reasons, or for those facing medical treatments like chemotherapy that could damage their ovaries. By freezing eggs at a younger age, women can stop the biological clock and use those eggs when they are ready to start a family.   

What to Expect After Treatment

Recovery varies by procedure.

  • After IUI: There is no recovery time. You can return to work immediately. You may have mild cramping.
  • After Egg Retrieval (IVF): You will need a day of rest. Bloating and cramping are common for a few days as the ovaries recover.
  • Ovarian Hyperstimulation Syndrome (OHSS): In rare cases, the ovaries overreact to medication, causing fluid buildup and pain. We monitor you closely to prevent this.
  • The Two-Week Wait: The time between treatment and the pregnancy test is often the hardest emotionally. We provide support resources during this waiting period.
