DHI Hair Transplant Planning and Procedure

The DHI hair transplant procedure starts long before you arrive in the operating room. Your medical team first evaluates your hair loss pattern, donor area strength, and long‑term goals to decide whether DHI is the best technique for you. Once DHI is selected, your surgeon carefully plans the number of grafts, the shape of your hairline, and the density for each zone of your scalp.

On the procedure day, the team follows a structured process that typically includes preparation, anesthesia, graft extraction, and Choi pen implantation. Most DHI hair transplant sessions last around 6–8 hours, depending on the number of grafts and individual needs.

How Is a DHI Hair Transplant Plan Created?

dhi-hair-transplant

A DHI hair transplant plan is created during a detailed consultation where your doctor evaluates your hair loss pattern, medical history, expectations, and donor area strength. The goal is to design a safe, natural-looking result that also protects your donor hair for the future.

Planning usually includes:

  • Assessing hair loss stage and donor area capacity
  • Designing the hairline and identifying priority zones (hairline, temples, crown)
  • Estimating graft needs, often around 2,000–3,500 grafts
  • Deciding whether DHI alone or DHI combined with FUE is more suitable

What Happens Before the DHI Procedure Begins?

dhi-hair-transplant

On the day of the procedure, preparation focuses on comfort and safety. After confirming your plan and answering final questions, the team may trim or shave the donor area and, depending on your case, part or all of the recipient area. The surgeon then marks your planned hairline and treatment zones directly on the scalp to guide the procedure.

Local anesthesia is applied to both the donor and recipient areas so you remain comfortable throughout the session. Some centers also offer light sedation in addition to local anesthesia, which can help you relax during a long procedure while still allowing you to breathe on your own. Once anesthesia takes effect, you should feel pressure and vibration but not sharp pain.

How Are Grafts Collected in DHI Hair Transplant?

In DHI, hair grafts are extracted individually from the donor area using a fine punch tool, with each graft containing one to four hairs. The grafts are carefully preserved in a special solution until implantation, while the medical team works in a controlled manner to protect donor density and ensure natural healing. Single-hair grafts are typically used to design the hairline, while multi-hair grafts help build overall density, with the total number of grafts planned in advance based on donor capacity, often between 2,000 and 3,500 grafts.

How Does the Choi Pen Implantation Step Work?

The Choi implanter pen is the key tool used in DHI hair transplantation, allowing each graft to be implanted gently and precisely in a single motion. After extraction, grafts are carefully loaded into the pen and placed into the scalp with full control over angle, depth, and direction, which helps create a natural-looking hairline and balanced density. Multiple pens and needle sizes are used throughout the procedure to maintain consistency and protect the grafts, and because the channel opening and implantation happen at the same time, grafts spend less time outside the body, supporting safe healing and reliable long-term results.

How Long Does a DHI Hair Transplant Take?

A DHI hair transplant typically takes around 6–8 hours, depending on the number of grafts, hair type, and donor area quality. This time includes preparation, local anesthesia, careful graft extraction, short breaks, and precise implantation using the Choi pen. Because each graft is placed individually with a high level of control, DHI may take slightly longer than a similar FUE procedure. To maintain safety and quality, surgeons usually limit the number of grafts in one session and will clearly explain the expected duration and plan before your procedure.

What Are the Advantages and Limitations of the DHI Procedure?

DHI hair transplant offers several advantages, especially for patients seeking natural detail and controlled density. Using the Choi implanter pen, surgeons can place each graft with precision, making the technique particularly effective for hairline refinement and focused density enhancement. The approach is designed to balance accuracy, safety, and long-term results rather than simply maximizing graft numbers.

Key advantages of DHI include:

  • Precise control over hair angle, direction, and depth
  • Natural-looking hairline design
  • Ability to place grafts closer together in selected areas
  • Reduced time grafts spend outside the body

Points to consider when planning DHI:

  • Sessions usually involve a moderate graft count to maintain precision
  • The technique requires an experienced surgical team and specialized tools
  • In some cases, FUE or a combined approach may be more efficient

Your surgeon will recommend the most suitable technique based on your hair loss pattern, donor capacity, and aesthetic goals, ensuring a personalized and reliable treatment plan.

FREQUENTLY ASKED QUESTIONS

How many grafts can be transplanted with DHI in one session?

DHI sessions usually involve around 2,000–3,000 grafts. In selected cases, up to about 3,500 grafts may be possible. Larger areas are often treated with FUE or planned in stages.

DHI is performed under local anesthesia, so pain is minimal. Most patients feel pressure rather than pain, and mild soreness afterward is usually easy to manage.

The donor area is commonly shaved, but partial or limited shaving may be possible depending on your case. Fully unshaven options are usually reserved for small areas.

We design the hairline based on your facial structure, age, and hair loss pattern. The plan is drawn and reviewed with you before the procedure.

A DHI session typically lasts about 6–8 hours and is done on an outpatient basis, so you can return to your hotel the same day.Advanced medical testing, such as an ECG (Electrocardiogram) or specialized coagulation tests, is usually required only if you are over the age of 45 or have a history of heart conditions, diabetes, or bleeding disorders.
