Designing the Eyebrow Shape

The morning of the procedure usually begins with the most critical artistic step: drawing the eyebrows. The surgeon and the patient work together in front of a mirror to outline the new brow shape. This is not a rough sketch; it is a precise blueprint that the surgeon will follow during implantation.

The design takes into account the patient’s facial features, bone structure, and gender. For men, the brow is typically lower, flatter, and thicker. For women, the design often includes a subtle arch that peaks at a specific point relative to the eye. The surgeon uses calipers and rulers to ensure symmetry and balance. Great care is taken to harvest the grafts intact. If the root is transected (cut), the hair will not grow. The surgeon extracts these follicles one by one. For an eyebrow transplant, the number of grafts required usually ranges from 200 to 600 per eyebrow, depending on the desired density. This process can take one to two hours. Creating the Recipient Sites

The final stage is the placement of the grafts in the prepared incisions. This is often done by the surgeon or highly trained technicians using fine forceps. It is a delicate process that requires steady hands and patience. To keep the hair bulb from breaking, each graft is handled carefully.

The placement follows a “cross-hatching” or “herringbone” pattern in the middle of the brow to mimic the way natural eyebrow hairs converge. The finest, single hairs are placed at the outer edges and the very top of the brow to create a soft transition. Thicker hairs, if any, are placed in the central body of the brow for density. The team constantly checks their work against the drawn design to ensure the shape is preserved.

Immediate Post-Procedure Protocol

When the last hair is placed, the procedure is complete. The brow area is usually left open to the air to heal or covered very loosely. The donor area at the back of the head is bandaged to keep it clean. The patient is given a mirror to see the immediate results.

Although the area will be red and possibly slightly swollen, the shape of the new brows is instantly visible. The medical team provides a detailed care kit, often including saline spray to keep the grafts moist, pain relievers, and instructions on how to sleep. The patient is monitored for a short period to ensure they feel well before being discharged to go home.
