Overview and Definition

What is Hand Surgery?

Hand Surgery is a highly specialized field of medicine that deals with both surgical and non-surgical treatment of conditions and injuries in the hand and upper extremity. Because the hand is an intricate map of small bones, delicate tendons, and sensitive nerves, any intervention requires an extreme level of anatomical knowledge. The goal of this specialty is to restore the complex mechanics of the hand, allowing patients to perform everything from heavy lifting to fine motor tasks like writing or playing a musical instrument.

The Precision of Hand and Microsurgery

At the pinnacle of this field is Hand and Microsurgery. This discipline involves the use of high-powered operating microscopes and miniaturized instruments to repair structures that are too small to be seen clearly by the naked eye. In cases of severe trauma, such as severed fingers or crushed limbs, Hand and Microsurgery is used to reconnect tiny blood vessels and nerve fibers, often thinner than a human hair. At Liv Hospital, we combine this technical precision with a deep understanding of hand aesthetics to ensure both functional and cosmetic success.

Injury and Causes

Common Causes of Hand Dysfunction

ORTHOPEDIC

The need for Hand Surgery often arises from the repetitive stress of modern life or sudden accidents. Overuse conditions, such as tendonitis or nerve entrapment, can cause chronic pain and a loss of grip strength. Furthermore, the hand is frequently the first part of the body injured during falls or workplace accidents. Whether the injury is a “simple” fracture or a complex ligament tear, the structural integrity of the hand must be meticulously addressed to prevent long-term stiffness.

Severe Trauma and Biological Repair

ORTHOPEDIC

In cases of catastrophic injury, Hand and Microsurgery becomes a life-changing intervention. Crushing injuries or industrial accidents can damage multiple tissue layers—bone, muscle, and skin—simultaneously. Without the ability to revascularize these tissues through microsurgical techniques, many patients would face permanent loss of limb function. At Liv Hospital, we specialize in “replantations” and “tissue transfers,” using the body’s own biological resources to rebuild what has been lost.

Diagnosis and Imaging

The Clinical Diagnostic Process

A successful Hand Surgery starts with a high-precision diagnosis. Because the hand has 27 bones and dozens of tendons, pinpointing the exact source of pain is essential. At Liv Hospital, our specialists perform detailed sensory mapping and strength testing. We check for “Tinel’s sign” and “Phalen’s maneuver” to identify nerve compressions, ensuring that the surgical plan targets the true “pain generator.”

Advanced Imaging for Hand and Microsurgery

To visualize the intricate structures of the hand, we utilize state-of-the-art imaging:

  • High-Resolution Ultrasound: Used to see tendons in motion and identify small cysts or “trigger” points.
  • MRI and MRA: Essential for evaluating ligament tears and planning Hand and Microsurgery by mapping out the blood vessels (angiography).
  • Electromyography (EMG): A “nerve test” used to confirm the severity of conditions like Carpal Tunnel Syndrome before proceeding with surgery.

Treatment and Recovery

Surgical Interventions and Microsurgical Repair

During Hand Surgery, the surgeon may perform a variety of tasks, from stabilizing a bone with tiny pins to releasing a tight ligament. In the case of Hand and Microsurgery, the procedure involves the use of a microscope to suture vessels that are 1mm or smaller. This level of detail ensures that even after a severe injury, the hand receives the blood supply and nerve signals necessary for healing.

The Recovery Timeline

Recovery from hand procedures is a delicate balance between protection and movement.

  • Weeks 1-2: Focus on wound healing and managing swelling; the hand may be protected in a custom-molded splint.
  • Weeks 3-6: Gradual introduction of “tendon gliding” exercises to prevent internal scarring.
  • Months 3+: Strengthening and a return to heavy lifting. For complex Hand and Microsurgery, nerve regeneration is a slow process that can continue for up to a year.

Rehabilitation Tips

Essential Rehabilitation Tips

The most critical part of Hand Surgery is the post-operative rehabilitation. Because the hand is prone to stiffness, working with a specialized Hand Therapist is essential. At Liv Hospital, our therapists provide custom-made splints and exercise programs designed to maximize your range of motion while protecting the surgical repair.

Long-Term Hand Care

  • Tendon Gliding: Perform your prescribed finger slides daily to ensure your tendons don’t “stick” to the surrounding tissue.
  • Edema Management: Keep your hand elevated above your heart during the first week to prevent the swelling that causes stiffness.
  • Scar Massage: Once the incision is healed, gentle massage with Vitamin E oil can help soften the scar and improve flexibility.
  • Desensitization: If you had nerve repair in Hand and Microsurgery, rubbing different textures on your skin can help “retrain” your brain to recognize touch.

FREQUENTLY ASKED QUESTIONS

What is a "Hand Therapist"?

A specialized physical or occupational therapist who focuses exclusively on the complex rehabilitation of the hand and wrist.

Hand surgeons are trained to make incisions that follow the natural creases of the hand. This helps camouflage the scar. While all surgery leaves a mark, scars on the hand often fade very well over time.

It depends on the surgery. For carpal tunnel release, you can use your hand for light activities immediately. For tendon repairs or fractures, you may need to keep the hand immobile in a splint for several weeks to allow healing.

Many hand surgeries can be done with “wide awake” anesthesia (WALANT). This uses local numbing medicine without an IV or sedation. You can talk to the surgeon during the procedure and drive yourself home afterward.

Soft tissues like skin heal in two weeks. Bone takes about 6 weeks. Nerves can take months to recover. Full return to heavy activities or sports typically takes 3 months or more depending on the injury.
