The Timeline of Settling

Facial augmentation results are not instantaneous. The “settling” process involves resolving swelling and relaxing the soft tissues over the new framework. While most acute swelling is gone by 2 weeks, residual edema can persist for months.

For implants, the final contour is typically visible around 3 to 4 months. For fat grafting, the final volume is assessed at 3 to 6 months, once the body has reabsorbed the non-viable fat cells. Patients must be patient and trust the healing process.

  • Resolution of acute and chronic edema
  • Relaxation of facial muscles over implants
  • Stabilization of fat graft volume
  • Timeline of 3 to 6 months for final contour
  • Psychological adjustment to the new look

Implant Stability and Integration

PLASTIC SURGERY

Over time, the body forms a capsule of scar tissue around solid implants. This capsule holds the implant in place. Porous implants allow tissue to grow into them, creating an even stronger bond.

Once healed, facial implants are very stable. They do not move with facial expressions or gravity. They become a structural part of the face, indistinguishable from the underlying bone to the casual observer.

  • Formation of the peri-prosthetic capsule
  • Tissue ingrowth into porous materials
  • Long-term fixation and immobility
  • Structural integration with the skeleton
  • Resistance to gravitational displacement

Monitoring for Complications

While rare, late complications can occur. Infection can develop months or years later, often triggered by dental work or trauma. Patients with facial implants should inform their dentist before procedures so prophylactic antibiotics can be prescribed.

Implant displacement or asymmetry can sometimes become apparent after all swelling has resolved. Regular follow-up appointments allow the surgeon to monitor the position and health of the implants.

  • Surveillance for late-onset infection
  • Antibiotic prophylaxis for dental procedures
  • Assessment of long-term symmetry
  • Monitoring for bone resorption
  • Identification of implant malposition

Fat Grafting Retention

Fat grafting results are generally permanent, but the volume can fluctuate with changes in body weight. If the patient gains significant weight, the face may become fuller. If they lose weight, the face may lose volume.

Maintaining a stable weight is key to preserving the results. Because not all transferred fat survives, a second touch-up session is sometimes planned to achieve the desired level of perfection after the first round has settled.

  • Permanence of surviving adipose cells
  • Volume fluctuation with BMI changes
  • Importance of weight stability
  • Potential for secondary touch-up sessions
  • Natural aging of the grafted fat

Longevity of Injectables

For patients who choose fillers, maintenance is an ongoing process. Hyaluronic acid fillers typically last 6 to 18 months, depending on the product and the area treated. High mobility areas like the lips dissolve faster than static areas like the temples or jawline.

Biostimulators like Poly L Lactic Acid last longer, up to 2 years, as they rely on the patient’s own collagen. Patients should establish a regular schedule with their injector to maintain the results without significant fluctuations in appearance.

  • Duration variability of HA fillers
  • Metabolic breakdown of injectable products
  • Longer half-life of biostimulatory agents
  • Scheduling of maintenance injections
  • Avoidance of volume stacking overfilling

Skin Care and Aging

Augmentation addresses structure, but skin care addresses the envelope. To maintain the best aesthetic result, patients should adhere to a strict skincare regimen. This includes sun protection to prevent collagen breakdown and topical treatments like retinoids to maintain elasticity.

As the face ages, the skin may relax over the implants. Non-surgical treatments like laser resurfacing or microneedling can help keep the skin tight and smooth, complementing the structural augmentation.

  • Daily application of broad-spectrum SPF
  • Use of medical-grade topical retinoids
  • Periodic collagen induction therapies
  • Prevention of photoaging
  • Synergy between structure and skin quality

Revision Surgery

In some cases, a patient may wish to change the size or shape of their implant after living with it for a while. Implants can be exchanged for larger or smaller sizes or removed entirely.

Revision surgery is more complex due to scar tissue, but it is a standard part of facial plastic surgery practice. Fat grafting can also be added later to soften the edges of an implant if the patient loses facial fat with age.

  • Exchange of implants for size adjustment
  • Removal of implants if desired
  • Management of capsule scar tissue
  • Corrective procedures for asymmetry
  • Adjunctive grafting for contour refinement

Psychological Adaptation

Getting used to a new face takes time. The brain has an internal map of what one looks like, and changes in structural features disrupt this map. Patients may experience a period of adjustment in which they feel they look “different,” but not necessarily “better,” until they acclimate.

Eventually, the new features become integrated into the patient’s self-image. The boost in confidence and the harmony of the features lead to high long-term satisfaction rates.

  • Adaptation of the internal body image
  • Period of cognitive dissonance
  • Integration of new features into identity
  • Long-term psychological satisfaction
  • improved social confidence

Dental Health Maintenance

For chin and jaw implants, maintaining excellent oral health is crucial. Gum disease or tooth decay can spread bacteria to the implant pocket. Regular dental check-ups and hygiene are mandatory to protect the investment in the face.

  • Adherence to rigorous oral hygiene
  • Prevention of periodontal disease
  • Regular professional dental cleanings
  • Prompt treatment of oral infections
  • Protection of the intraoral incision site

Lifestyle Factors

Smoking and excessive alcohol consumption accelerate facial aging and can compromise the health of the tissues surrounding the augmentation. A healthy lifestyle, including adequate sleep, nutrition, and hydration, supports the longevity of aesthetic results.

  • Avoidance of smoking and nicotine
  • Moderation of alcohol intake
  • Impact of sleep hygiene on appearance
  • Nutritional support for tissue health
  • Holistic approach to anti-aging

FREQUENTLY ASKED QUESTIONS

Will the implants need to be replaced?

Facial implants are solid and durable. Unlike breast implants, they do not have a shell that can rupture or leak. They are designed to be permanent and typically do not need to be replaced unless you want to change the size or if an infection occurs.

There is a phenomenon called bone resorption, where the pressure of the implant causes a slight indentation in the underlying bone. This is usually minor, self-limiting, and does not affect the stability of the implant or the health of the bone in the vast majority of cases.

Yes, absolutely. In fact, having facial implants often improves the results of a future facelift. The implants provide a strong structure for the facelift to redrape the skin over, usually leading to a more defined, longer-lasting result.

After the initial healing period (about 6 weeks), the implants are very stable. However, a severe trauma that would break a natural bone could also shift an implant or damage the surrounding tissue. You should protect your face during contact sports.

The grafted fat becomes part of your facial tissue. It will age at the same rate as the rest of your face. It does not sag any more than your natural fat would. Gravity eventually affects all tissues, but the added volume generally keeps the face looking younger for longer.
