Circumcision Overview and Definition

What is Circumcision ?

Circumcision

Circumcision, known medically and culturally across the globe for millennia, is the surgical removal of the foreskin, the retractable fold of skin that covers the head (glans) of the penis. While it is one of the most common surgical procedures performed worldwide, it is a subject that encompasses medical, religious, cultural, and personal dimensions. At Liv Hospital, circumcision is done with great care and attention, making sure that this important family event is treated with the same precision as a major surgery and the understanding needed for children.

The procedure is most commonly performed on newborn infants for religious or traditional reasons, particularly in Jewish and Islamic cultures. However, it is also performed for strictly medical reasons in children and adults. From a biological standpoint, the foreskin is a special type of tissue that shields the glans; however, taking it off has been linked to several long-term health advantages, such as a lower chance of urinary tract infections, some sexually transmitted infections, and penile cancer. At Liv Hospital, we utilize advanced surgical techniques ranging from traditional dissection to modern thermal cautery and “sutureless” methods to ensure minimal discomfort and rapid healing.

The Evolution of the Procedure

The Procedure in a Clinical Setting

Historically, circumcision was performed without anesthesia and with rudimentary tools. Today, the landscape has changed entirely. At Liv Hospital, the safety and comfort of the patient whether a two day old infant or a thirty year old adult are the primary focus. Modern circumcision is performed in a sterile operating environment using local, regional, or general anesthesia. The goal is to remove the prepuce (foreskin) while preserving the aesthetics of the penis and ensuring that the frenulum (the sensitive tissue on the underside) is handled with care.

Circumcision Symptoms and Risk Factors

When Circumcision Becomes Necessary?

While many circumcisions are “elective,” several medical conditions make the procedure a clinical necessity. Phimosis, a condition where the foreskin is too tight to be retracted, can lead to pain, scarring, and difficulty urinating. Another condition, Balanitis, involves chronic inflammation or infection of the glans, often caused by poor hygiene or underlying health issues like diabetes. Furthermore, infants with recurrent Urinary Tract Infections (UTIs) or structural abnormalities of the urinary tract are often referred for circumcision as a preventive measure.

Circumcision Diagnosis and Tests

Pre-Surgical Evaluation

Before any surgical intervention at Liv Hospital, a thorough diagnostic phase is required. For newborns, the first step is often a physical examination to ensure the baby is healthy and that there are no anatomical issues like Hypospadias (where the urinary opening is not at the tip), which would require the foreskin for later reconstructive surgery. For adults and older children, blood tests, coagulation profiles, and physical assessments of the inflammatory site are essential to ensure the patient is fit for anesthesia.

Circumcision Treatment and Care:

Surgical Excellence

The “treatment” phase is the surgical act itself. At Liv Hospital, we offer several modalities tailored to the patient’s age. For infants, the use of specialized clamps (like the Gomco or Mogen clamp) allows for a quick, bloodless procedure. For older children and adults, we perform a surgery that removes the tissue, often using Laser or Electrocautery to reduce bleeding and using stitches that dissolve on their own for a neat appearance. The post-operative care involves pain management and specific hygiene protocols to prevent infection.

Recovery and Follow-up:

Ensuring Long-term Success

The success of a circumcision is determined in the weeks following the surgery. Healing typically takes 7 to 10 days for infants and slightly longer for adults. Liv Hospital provides a comprehensive follow-up schedule to monitor the healing of the surgical site, ensure there is no significant swelling or infection, and guide the patient through the stages of skin maturation at the incision line. Understanding the difference between normal postoperative redness and a complication is key to a stress-free recovery.

10 Essential Insights into Circumcision

  1. Global Prevalence: Approximately 37% to 39% of men worldwide are circumcised, with the highest concentrations in the Middle East, North Africa, and the United States.

  2. Medical Indications: Beyond tradition, conditions like Paraphimosis (where the foreskin gets stuck behind the glans) are surgical emergencies that require circumcision.

  3. STI Protection: The World Health Organization (WHO) has recognized circumcision as a significant factor in reducing the female-to-male transmission of HIV by approximately 60%.

  4. Penile Cancer Prevention: Men who are circumcised have a significantly lower risk of developing squamous cell carcinoma of the penis, largely due to better hygiene and fewer HPV infections.

  5. The “Hypospadias” Warning: Circumcision should never be performed on a child with hypospadias until a urologist evaluates them, as the foreskin is needed for repair.

  6. Pain Management: Modern anesthesia techniques, including EMLA cream for infants and dorsal penile nerve blocks for adults, have made the procedure virtually painless during the operation.

  7. Cosmetic Outcomes: A well-performed circumcision results in a smooth, symmetrical appearance. Surgical precision at Liv Hospital ensures that the “inner mucosa” and “outer skin” are perfectly aligned.

  8. The Role of Smegma: In uncircumcised males, smegma (a collection of skin cells and oils) can accumulate under the foreskin; if not cleaned, it can lead to infection, which is avoided after circumcision.

  9. Healing Time: In newborns, the surgical site often heals so quickly that the plastic ring or bandage falls off naturally within a week.

  10. Cultural Sensitivity: At Liv Hospital, we respect the cultural significance of the procedure while adhering to the strictest medical safety standards, bridging the gap between tradition and science.

FREQUENTLY ASKED QUESTIONS

At what age is it best to perform circumcision?

From a medical standpoint, the newborn period (the first month) is often preferred because the skin is very thin, healing is rapid, and the psychological impact on the child is non-existent. However, it can be safely performed at any age.

This is a common concern. Most large-scale medical studies indicate that there is no significant difference in sexual satisfaction or function between circumcised and uncircumcised men.

For infants, local anesthesia and a “nerve block” are usually sufficient. For older children who might be anxious or unable to stay still, general anesthesia is often used at Liv Hospital to ensure the highest safety and surgical precision.

Like any surgery, risks include minor bleeding or local infection. However, in a controlled hospital environment like Liv Hospital, these risks are extremely low (less than 1%).

Most adults can return to desk work within 2 to 3 days. However, strenuous physical activity and sexual intercourse should be avoided for 4 to 6 weeks to ensure complete internal healing.
