How Is Prostatitis Diagnosed at Liv Hospital?

Diagnosing prostatitis in no longer a matter of “guesswork and antibiotics.” At Liv Hospital, we recognize that many patients arrive frustrated after multiple failed treatments. Our diagnostic paradigm is built on the principle of Phenotyping. Because prostatitis symptoms can arise from the prostate gland, the bladder, the pelvic floor muscles, or the central nervous system, we use a battery of tests to pinpoint exactly which “systems” are malfunctioning. This ensures that a man with muscle-based pain isn’t incorrectly treated with months of unnecessary antibiotics.

What Is the Segmented Urine Culture Test for Prostatitis at Liv Hospital?

The “Gold Standard” for localizing infection in the prostate is the segmented urine culture.

The Process: We collect urine samples at different stages of the voiding process: the initial stream (urethral), the mid-stream (bladder), and then—following a specialized prostate massage—the expressed prostatic secretions (EPS) and a post-massage urine sample.
Localization: By comparing bacterial counts across these samples, our lab at Liv Hospital can determine if the infection is truly inside the prostate or merely in the bladder or urethra. For many patients, we use the simplified 2-Glass Pre- and Post-Massage Test (PPMT), which is equally effective and more efficient.

UPOINT Clinical Phenotyping

The most critical diagnostic tool at Liv Hospital is the UPOINT system. This is a clinical classification used to categorize each patient into one or more of six domains:
U (Urinary): Assessment of flow rates and residual urine.
P (Psychosocial): Evaluating the impact of stress, anxiety, or depression on pain perception.
O (Organ-Specific): Evaluating the prostate itself for tenderness or inflammation.
I (Infection): Ruling in or out active bacterial colonization.
N (Neurological/Systemic): Checking for pain outside the pelvis or irritable bowel symptoms.
T (Tenderness): Palpating the pelvic floor muscles for “trigger points” or knots. By “scoring” a patient in each domain, we create a multidimensional treatment map.

Advanced Microbiology and DNA Sequencing

Standard urine cultures often fail to grow the slow-growing or “fastidious” bacteria that can hide in the prostate.

PCR and Next-Gen Sequencing (NGS): At Liv Hospital, we utilize advanced DNA sequencing to identify bacterial DNA even when the bacteria won’t grow in a traditional petri dish.
Biofilm Analysis: We check for the presence of “biofilms”—slimy protective layers that bacteria build inside the prostatic ducts to hide from antibiotics—helping us understand why previous treatments may have failed.

Multiparametric MRI (mpMRI) and Ultrasound

Imaging plays a vital role in ruling out structural complications that can mimic or complicate prostatitis.

Identifying Abscesses: In acute cases, an MRI can detect a prostatic abscess (a pocket of pus) that requires surgical drainage.
Calcification Mapping: Chronic inflammation often leads to “prostatic stones” (calcifications). While these are usually benign, large stones can harbor bacteria and lead to recurrent infections.

Digital Rectal Examination (DRE) and EPS Analysis

During the physical exam, the urologist performs a DRE to assess the prostate’s physical state.

Acute Sensitivity: In Category I prostatitis, the prostate is exquisitely tender and “hot” to the touch.
Expressed Prostatic Secretions (EPS): In chronic cases, the fluid expressed during a gentle massage is examined under a microscope at Liv Hospital for white blood cells (leukocytes). A high white cell count confirms the presence of inflammation, even if no bacteria are found.

Uroflowmetry and Post-Void Residual (PVR)

Since prostatitis often causes urinary “frequency” and “hesitancy,” we must measure the actual performance of the bladder.

Flow Analysis: The patient urinated into a flow meter to check for obstruction caused by a swollen prostate.
Emptying Efficiency: We use a non-invasive ultrasound to check if the patient is retaining urine, which could be a source of constant irritation or a sign of “high-pressure” voiding that forces urine back into the prostate.

Pelvic Floor Trigger Point Assessment

For the majority of men with Category III (CPPS), the “test” isn’t in a lab, but in the muscles.

Internal Palpation: Our specialized therapists at Liv Hospital check the levator ani and obturator internus muscles for “trigger points.” These are hyper-irritable spots that, when pressed, radiate pain to the penis, scrotum, or rectum.
Diagnosis of Myofascial Pain: Identifying these points confirms that the pain is muscular (myofascial) rather than an active infection, shifting the treatment focus to physical therapy.

Cystoscopy (In Specific Cases)

If a patient has blood in their urine or if the pain is localized specifically to the urethra, we may perform a Cystoscopy.

Direct Visualization: A thin, flexible camera is used to look inside the urethra and bladder.
Ruling out Mimics: This test ensures that the “prostatitis” symptoms aren’t actually being caused by a urethral stricture, a bladder stone, or interstitial cystitis (painful bladder syndrome).

Why do I need a prostate massage for the diagnosis?

It helps “squeeze” the prostatic fluid into the urethra so it can be collected in a urine sample. This fluid contains the most accurate information about bacteria and white blood cells inside the gland.

Can my PSA be high if I only have prostatitis?

Yes. Inflammation irritates the prostate cells, causing them to leak PSA into the blood. At Liv Hospital, we wait for the inflammation to clear before re-testing to get your true baseline.

Is an MRI necessary for every case?

Not for every case. We typically use it if symptoms are severe, if we suspect an abscess, or if your PSA is elevated and we need to rule out cancer.

What if all my tests come back "negative" but I'm still in pain?

This is common in Category III CPPS. It usually means the problem is in the pelvic muscles or the nerves. At Liv Hospital, a “negative” culture is actually a helpful clue that points us toward physical therapy or nerve modulation.
