Treatment and Rehabilitation

The treatment for profound hearing loss involves two major phases: the surgical implantation of the electronic device and the long-term, intensive auditory rehabilitation. The Cochlear Implant itself is a major surgical intervention, but its success relies heavily on the post-surgical therapy. Multidisciplinary approach to ensure the best possible hearing and communication outcomes.

Medical Treatment Options

EAR NOSE THROAT

Before surgery, medical options are explored to ensure the maximum benefit from non-surgical intervention. For most implant candidates, traditional medical treatments and devices have already failed to provide adequate hearing.

 Trial of Amplification

Before an implant is considered, every candidate must undergo a trial period with powerful hearing aids. This ensures that the hearing loss is truly severe or profound and cannot be adequately managed by making sound louder.

  • Hearing Aids: These devices amplify sound that travels through the outer and middle ear to the inner ear. If the inner ear damage is too severe, amplification is not enough.
  • Non-Implant Alternatives: For some specific types of hearing loss, bone conduction devices (which transmit sound through bone vibration) may be considered, but these are not the solution for sensorineural loss.

 Pre-Surgical Medical Care

Any underlying ear infections or diseases must be fully resolved before surgery can be safely performed. The surgical team will ensure the patient is medically stable and all risks, particularly those related to anesthesia, have been minimized.

The Surgical Intervention

EAR NOSE THROAT

 Cochlear Implant Procedure

The placement of the implant’s internal component is a specialized surgical intervention performed by an otologist (ear surgeon) under general anesthesia. The goal is to safely insert the delicate electrode array into the cochlea without causing further damage to the inner ear structures.

The procedure typically lasts between 2 to 4 hours. It involves making a small incision behind the ear, creating a well to place the receiver/stimulator, and then carefully threading the electrode array into the cochlea.

 Minimally Invasive Procedures

Modern cochlear implant surgery is considered a highly specialized, minimally invasive procedure compared to older approaches. LIV Hospital utilizes advanced surgical techniques designed to preserve any remaining natural hearing the patient may have (Residual Hearing Preservation).

  • Soft Surgery Techniques: Our surgeons use precise, controlled drilling and careful insertion tools. This gentle approach is key to minimizing trauma to the cochlea, which is especially important for Hybrid Implant candidates.
  • Advanced Imaging Guidance: Surgeons use the pre-operative CT and MRI images for highly accurate, real-time guidance during the delicate insertion phase.

Why Choose LIV Hospital

LIV Hospital’s Cochlear Implant program is defined by its multidisciplinary excellence and commitment to advanced care.

  • Our team includes internationally trained otologists and specialized audiologists who work together from evaluation to activation.
  • We prioritize early intervention in children, which is critical for their speech and language development, and tailor the surgery for optimal outcomes in the pediatric population.
  • We provide a dedicated Auditory Verbal Therapy (AVT) program staffed by certified speech pathologists, ensuring high-quality long-term rehabilitation.

Recovery Time and Expectations

The recovery process has two parts: physical recovery and auditory rehabilitation. Physical recovery from the surgery is typically quick.

  • Surgical Recovery: Patients usually stay in the hospital overnight. The external part of the incision heals within 1 to 2 weeks. Most daily activities can be resumed shortly after.
  • Implant Activation (Switch-On): The implant is typically activated, or “switched on,” about 2 to 4 weeks after surgery. This allows the incision to fully heal before the device is used.

Auditory Rehabilitation Programs

Auditory rehabilitation is the most critical component of the Cochlear Implant rehabilitation program. This phase teaches the brain how to interpret the new electrical signals as meaningful sounds.

  • Mapping Sessions: The audiologist uses specialized software to program (or “map”) the external sound processor. These are frequent in the first year to adjust the loudness and pitch levels for comfortable hearing.
  • Auditory-Verbal Therapy (AVT): This intensive therapy, especially crucial for children, focuses on developing spoken language through hearing. It is a long-term commitment that typically lasts several years.

Follow-up and Monitoring

 Long-term Monitoring Protocols

Long-term follow-up is necessary to ensure the device is functioning correctly and the patient is maximizing their listening potential.

  • Regular Mapping: Mapping sessions are required every few months in the first year, gradually reducing to yearly checks thereafter. These ensure the program settings are optimized for the patient’s changing needs.
  • Device Checks: The external sound processor and all components are checked annually for damage and to ensure they are using the latest technology and software.
  • Hearing Conservation: Patients are counseled on protecting the non-implanted ear (if applicable) and avoiding activities that could damage the external processor.

 Preventing Complications

While rare, a specific vaccination is required to prevent a serious complication. Patients must receive the pneumococcal vaccine before or shortly after the surgery, as it protects against meningitis, which is a slight risk after any inner ear procedure.

FREQUENTLY ASKED QUESTIONS

What are the treatment options for severe hearing loss?

The treatment options are either powerful traditional hearing aids (amplification) or the surgical option of a Cochlear Implant, which is chosen when hearing aids provide insufficient benefit.

The surgical recovery takes about 2 to 4 weeks before the implant is activated. Auditory rehabilitation, which involves mapping and therapy sessions, is a long-term commitment that usually spans several years, depending on the patient’s age and progress.

Yes. If your hearing loss is diagnosed as severe to profound sensorineural loss and traditional hearing aids are ineffective, you will need surgery to place the internal component of the Cochlear Implant.

After the surgery, only routine pain medication and antibiotics are typically used for a short period to manage discomfort and prevent infection. The Cochlear Implant itself is an electronic device, not treated with long-term medications.

You can expect a small incision behind the ear, which will be tender for a few days. The physical recovery is quick. The main change is the period after activation, where you will begin the process of hearing and learning to understand new electronic sounds.
